Be 19 years of age or older and have identification showing present address.

Be willing and able to invest the time and money required to provide proper care for the pet. Estimated minimum cost annually: $300 per dog, $225 per cat.

Be prepared to pay an adoption fee of $100.00 (check or cash only).

When you have completed the form, print a copy for your records by clicking on the Print Application button at the bottom of the form.
